In assessment, what is the reason for referral and the referral source, what special considerations do they make for that referral source (e.g., "We send their clients for urine screenings" or "We always call the agency on the day of testing to confirm that the client showed and did finish testing"), are there special timelines (e.g., "These reports go out in a week") or paperwork to fill out (e.g., "These evaluations include filling out a Psychiatric Review Technique form")

Who the client is and who gets the report, what is it used for, when is it due, what format is there to follow or special considerations that must be adhered to (e.g., "We always go through each area of disability that this handout lists; that makes it easier for the workers when they see this report to fill out forms to apply for services for the client")

Confidentiality and consent--remember that if the client is over age 13, you must obtain consent to release information, and any audio or videotaping requires additional forms, consent forms are valid for only one year if dated, if not dated only for one day, specific information about what will be released/requested, to whom, and for what specific purpose must be on the form

Special policies--how are hours logged, whom do you report to, where do you meet clients, test them, obtain test materials, store confidential records, are there policies on taking information home from the site?

Emergency Procedures--who do you go to if a client contacts you with an emergency, are you ever there without coverage and what do you do about client questions
